It is the practice of the Princeton Public Schools to require a current physical examination for all sixth grade students. In addition, all new students to the district and all athletes must submit a Physical Evaluation Form completed within the past 365 days.

If you wish to participate in a sport at JW, you should complete once per year* within 90 days of the first day of practice for the sport:

4. Read the NJSIAA memo regarding testing for steroid use, list of banned drug classes, then sign and return the consent form.
5. The New Jersey Department of Education's ANNUAL ATHLETIC PRE-PARTICIPATION PHYSICAL EXAMINATION FORM – which includes:

  • Preparticipation Physical Evaluation History Form (page 1) - completed by parent and student;
  • Supplemental History Form (page 2) - completed by parent and student;
  • Preparticipation Physical Examination Form (page 3) - completed by physician; and
  • Clearance Form (page 4) - completed by physician.
For subsequent sport seasons* during the same school year, simply complete within 90 days of the first day of practice for the sport:


* Note: Physical examination forms expire on the 365th day after the doctor's signature for students participating in sports.  To continue participating in a sport, students MUST submit a new physical evaluation form prior to the expiration.

Please update the health office with all emergency contact information.

*All completed forms must be submitted to the school nurse.
